Job Description

The position is for a nurse in a hospital-based skilled nursing facility that focuses on geriatric nursing. The facility provides supervised restorative care for older patients recovering from hospitalization or acute illness. Nurses play a critical role in bridging the gap between high-tech hospitalization and family home care, ensuring patients improve their health skills and functional abilities.

Responsibilities

• Provide nursing care to patients recovering from post-operative procedures such as hips, knees, and shoulders.
• Caring for patients with diagnoses including pneumonia and Congestive Heart Failure (CHF) requiring rehabilitation before home discharge.
• Work collaboratively with a multidisciplinary team to promote optimal patient outcomes.
• Monitor patient progress and report any changes to the charge nurse.

Required Experience / Certifications / Licensure

• Active nursing license in the state of West Virginia.
• Basic Life Support (BLS) certification required.
• Experience in geriatric nursing or post-acute care preferred.
