Job Description

We are seeking a skilled Occupational Therapist for home health care. The ideal candidate will be responsible for working with patients in various settings, providing comprehensive therapy for a range of conditions including cardiac rehabilitation, post-surgical recovery, and neurological care.

Responsibilities

• Conduct evaluations and assessments of patients' needs and conditions.
• Develop and implement individualized treatment plans focusing on activities of daily living.
• Provide education to patients and families regarding safety and injury prevention.
• Utilize TENS, ultrasound, joint mobilization, and splinting as appropriate.
• Document patient progress and maintain accurate billing and documentation.
• Facilitate home exercise programs and assist with lifting and transferring patients.
• Conduct discharge planning and community re-entry evaluations.

Required Experience / Certifications / Licensure

• Minimum of 1 year of experience in home health care; 2 years preferred.
• Must be a licensed Occupational Therapist in Florida or hold a compact license.
• Current BLS certification is required.
• OASIS training is preferred.
• Must possess valid driver's license and have access to a car for home visits.
• Candidates must be COVID vaccinated or have a valid exemption.
• Flu shot required unless a declination is submitted; masks must be worn at all times in the facility.
