Assignment Duration

This is a 13-week contract position.

Schedule

Day shift, 5 days a week (08:00 AM - 04:30 PM). This facility cannot accommodate 4x10s or 3x12s.

Job Description

We are seeking a dedicated and experienced Home Health Occupational Therapist (OT) to provide exceptional patient care in Medford, OR, and surrounding areas. As an OT, you will work with patients in their homes to facilitate rehabilitation and improve daily living skills.

Responsibilities

  • Deliver individualized occupational therapy services to clients in their homes.
  • Conduct assessments and develop treatment plans based on patients' needs.
  • Provide therapy sessions focused on enhancing patients' ability to perform daily activities.
  • Document patient progress and adjust treatment plans as necessary.
  • Collaborate with other healthcare professionals as part of the patient's care team.

Required Experience / Certifications / Licensure

  • Minimum 1 year of skilled home health experience required.
  • Certifications: BCLS (Basic Cardiac Life Support) required.
  • Experience with Oasis documentation is mandatory.
  • Traveler must have reliable transportation.
  • COVID vaccination series and one booster required.
