Schedule

Day Shift: 5x8-Hour (08:00 - 17:00). On-call rotation required on weekends.

Job Description

The Registered Nurse will provide compassionate home health and hospice care to patients, primarily focusing on geriatric patients with a life expectancy of six months or less. This role demands a high level of cultural awareness and proactive engagement in end-of-life care practices.

Responsibilities

• Conduct comprehensive intake assessments and ongoing evaluations.
• Administer medications and manage pain through evidence-based methodologies.
• Develop patient-centered care plans in collaboration with patients and families.
• Provide education and support to patients and families navigating end-of-life care.
• Conduct symptom assessments for conditions such as altered mental status, anorexia, and pain management.
• Support palliative care initiatives, including managing spiritual and emotional needs.

Required Experience / Certifications / Licensure

• Minimum of 2 years of nursing experience in Home Health/Hospice settings.
• Current RN license (NC or Compact license) in hand at the time of start.
• BLS certification required; additional certifications in End of Life Care are preferred.
• Experience with medication administration, pain management, and symptom assessment.
