Job Description

The Physical Therapist will be responsible for providing comprehensive physical therapy services to patients in accordance with federal and state regulations, and organizational policies. The role involves actively managing patient schedules, conducting assessments, and developing personalized plans of care to promote optimal functional independence and safety.

Responsibilities

• Monitor and amend patient visit schedules to maximize productivity.
• Perform comprehensive assessments of assigned patients, including psychosocial and safety factors.
• Develop and implement individualized plans of care with specific goals.
• Provide skilled physical therapy treatment and maintain quality of care.
• Coordinate patient care with clinical team members, ensuring effective communication.
• Supervise and direct clinical activities of PTAs, students, and aides.
• Complete necessary documentation according to regulatory standards.
• Process billing and ensure accuracy in insurance coverage documentation.
• Engage in community training and marketing of rehabilitation services.
• Perform various patient-related health assessments and procedures.
• Ensure timely communication of changes in patient care schedules.
• Participate in peer review audits and accept constructive feedback professionally.

Required Experience / Certifications / Licensure

• Current Physical Therapist license in accordance with state regulations.
• Experience in home health care and knowledge of OASIS documentation preferred.
• Strong communication and organizational skills.
