Job Description

Under the direct supervision of a physical therapist, the Physical Therapist Assistant carries out individualized treatment plans in compliance with federal and state regulations, organizational policies, and APTA guidelines. The role involves collaboration with physical therapists to develop care plans that promote optimal functional independence and patient safety.

Responsibilities

• Execute individualized treatment plans as per clinical documentation standards.
• Assist in the development of care plans to enhance patient functionality and safety.
• Provide skilled physical therapy interventions while assessing patient status and progress during each visit.
• Coordinate patient care with a multidisciplinary clinical team to ensure effective communication and optimal care.
• Report changes in patient conditions related to prescribed care plans in a timely manner.
• Maintain accurate clinical documentation reflecting care provided and outcomes achieved.
• Enhance personal knowledge of physical therapy standards and stay updated on HHCAHPS standards.

Required Experience / Certifications / Licensure

Candidates must possess a valid state license as a Physical Therapist Assistant.
