Job Description

The Outpatient Physical Therapist is responsible for evaluating patients and developing, implementing, and modifying treatment plans tailored to individual needs, goals, and physician directions. The therapist will delegate and direct specific therapy services, ensuring the design of functional treatment programs that facilitate a return to community activities. Responsibilities also include serving as an educator for patients, families, students, coworkers, and oneself, along with fulfilling other professional duties related to patient care and therapy service development.

Responsibilities

• Conduct comprehensive evaluations and create treatment plans focused on patient progress.
• Document care accurately, including assessments, goal writing, and charge entries.
• Participate in student programs as necessary, providing supervision and evaluations.
• Develop and pursue a personalized professional growth plan.
• Adhere to state practice acts and maintain professional licensure.
• Engage in performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• Minimum of 1 year outpatient clinical experience.
• Current CPR certification.
• Active applicable state licensure.
• Previous experience with Select Medical is a plus.
