Outpatient Physical Therapist - 6 Month Contract

Job Description

We are seeking a dedicated and skilled Outpatient Physical Therapist for a 6-month contract position. The therapist will evaluate assigned patients and develop, implement, and modify personalized treatment plans according to patient needs, goals, and physician directions. The role involves delegating and directing therapy services while designing functional treatment programs that facilitate the patient's return to community activities.

Responsibilities

• Conduct comprehensive evaluations for all assigned patients.
• Utilize clinical reasoning to create appropriate treatment plans and goals focused on progressing patients towards discharge.
• Document all medical records accurately, clearly, and in a timely manner, ensuring compliance with state, federal, and organizational guidelines.
• Ensure care is delivered under valid physician orders and within insurance approvals.
• Participate in student training programs, providing a supportive learning environment.
• Create and pursue a personal professional development plan in collaboration with clinical management.
• Adhere to the discipline-specific State Practice Act and maintain professional licensure in good standing.
• Actively engage in performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Bachelor’s or Doctorate degree in Physical Therapy from an accredited institution.
• 6 months to 1 year of outpatient clinical experience is required.
• Current CPR certification.
• Active and applicable state licensure as a Physical Therapist is required.
• Previous experience with Select Medical is preferred.

Schedule

This position follows a consistent 5x8.0 schedule.
