Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist will evaluate all assigned patients, developing, implementing, and modifying treatment plans according to the patients' needs and goals. The role involves overseeing discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that enable effective return to community activities. The therapist will also act as an educator for patients, families, students, and coworkers, performing various professional duties related to patient care and program delivery.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to establish appropriate treatment plans and goals aimed at successful discharge.
• Document care in a clear, concise, accurate, and timely manner in compliance with state, federal, and clinic policies.
• Provide care under signed, valid physician orders within insurance approvals.
• Participate in student programs, ensuring a positive learning environment through proper supervision and evaluation.
• Develop and pursue an individualized professional development plan.
• Maintain professional licensure in good standing and complete state-required continuing education.
• Engage in performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months to 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state license for Physical Therapy.
• Previous successful assignments with the agency highly desired.
