Job Description
We are seeking an experienced Physical Therapist Assistant (PTA) or Certified Occupational Therapy Assistant (COTA) for a home health position. The ideal candidate should be familiar with OASIS documentation and have experience in home care settings. This role may involve visits across a 60-mile radius within Contra Costa County, with an expected average of 6-7 visits per day depending on the types of visits required. Overtime must be pre-approved, and there is a cancellation policy of two weeks.
Responsibilities
- Conduct evaluations and develop treatment plans for patients.
- Administer therapy interventions based on established protocols.
- Document patient progress and treatment outcomes using Homecare Homebase (HCHB) software.
- Coordinate care with other healthcare professionals as needed.
- Assist patients with mobility and activities of daily living.
- Adhere to all safety and regulatory requirements while providing care.
Required Experience / Certifications / Licensure
- Home Health and OASIS experience required; no exceptions.
- Experience with Homecare Homebase documentation is mandatory.
- Consideration may be given to candidates without HCHB experience at a reduced rate for the first two weeks if training is provided.
