Job Description
The Registered Nurse (RN) in Home Health is responsible for providing skilled nursing care to patients in their homes, with a focus on rehabilitation, disease management, and education for patients and their families. The position requires home health experience and proficiency in OASIS documentation.
Responsibilities
- Conduct patient assessments and evaluations
- Perform skilled nursing interventions and treatments
- Educate patients and families on disease management and home exercise programs
- Document patient care and interventions accurately in compliance with OASIS standards
- Manage medication administration and monitoring
- Utilize adaptive devices and equipment for patient mobility and safety
- Provide education on safety and injury prevention
- Identify signs and symptoms of physical, sexual, and psychological abuse
Required Experience / Certifications / Licensure
- Minimum of 1 year of nursing experience in Home Health
- BLS certification required
- Valid Florida RN license or compact license
- OASIS training preferred
- Must be COVID vaccinated or have an approved exemption
- Flu shot or declination required
- Must possess a valid driver's license and have a reliable vehicle for home visits
