Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist is responsible for evaluating all assigned patients, developing, implementing, and modifying treatment plans in accordance with the patient's needs, goals, and physician direction. This role involves the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs to facilitate an effective return to community activities. The therapist serves as an educator for patients, families, students, coworkers, and themselves while upholding the organization's mission and core values.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Develop appropriate treatment plans and goals aimed at discharging patients within an established length of stay.
• Document medical records in a clear, concise, and timely manner, ensuring compliance with state, federal, and organizational guidelines.
• Participate in student programs, providing a positive learning environment and appropriate supervision.
• Create an individualized professional development plan in collaboration with clinical management.
• Adhere to the discipline-specific State Practice Act and maintain professional licensure in good standing.
• Engage in performance improvement initiatives and contribute feedback to maximize outcomes.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ years of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure for physical therapy.

Schedule

This is a 5x8 schedule with shifts of 8 hours.

Assignment Duration

This is a 13-week contract position.
