Occupational Therapist

Job Description

Caledonia Home Health Care & Hospice is seeking a dedicated Occupational Therapist to provide in-home therapy services aimed at maximizing the independence and safety of our home care and hospice patients. The successful candidate will be expected to work independently while also collaborating effectively as part of a multidisciplinary team.

Responsibilities

• Provide in-home occupational therapy services.
• Develop and implement treatment plans tailored to individual patient needs.
• Conduct evaluations and monitor patient progress toward achieving therapy goals.
• Collaborate with a multidisciplinary team to ensure cohesive patient care.
• Educate patients and families on therapeutic interventions and safety measures.

Required Experience / Certifications / Licensure

• Current Vermont licensure as an Occupational Therapist.
• Valid driver’s license and reliable vehicle for travel to patient homes.
• Experience in a home care setting is preferred.

Schedule

This position requires availability for up to 40 hours per week, Monday through Friday.

Assignment Duration

This is a 13-week contract position.
