Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist is responsible for evaluating assigned patients and developing, implementing, and modifying treatment plans based on patient needs, goals, and physician direction. This role involves delegating and directing all discipline-specific therapy services for assigned patients, designing functional treatment programs that facilitate a return to community activities, and serving as an educator for patients, families, students, and colleagues. The therapist also participates in professional development and ensures compliance with regulatory standards and organizational policies.

Responsibilities

• Conduct comprehensive evaluations of assigned patients to develop individualized treatment plans.
• Document medical records accurately and timely in compliance with state, federal, and clinic policies.
• Provide care under signed, valid physician orders and within insurance approval.
• Participate in student programs by supervising and evaluating their learning progress.
• Create and pursue a professional development plan in collaboration with clinical management.
• Adhere to state practice acts and maintain professional licensure in good standing.
• Engage in organizational performance improvement initiatives to enhance quality of services.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• Minimum of 1 year of outpatient clinical experience.
• Currently holds a CPR certification.
• Active and applicable state licensure for physical therapy.

Schedule

This is a 5x8.0 schedule, with immediate availability required. No new graduates, please.

Assignment Duration

This is a 13-week contract position.
