Job Description

The Outpatient Physical Therapist is responsible for evaluating all assigned patients and developing, implementing, and modifying treatment plans according to patient needs, goals, and physician direction. This role also includes the delegation and direction of discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs to facilitate a return to community activities.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals.
• Document medical records clearly, accurately, and in a timely manner, including goal writing and charge entry.
• Ensure care is provided under valid physician orders and within insurance approval as indicated.
• Participate in student programs by providing a positive learning environment and appropriate supervision.
• Create an individualized professional development plan and pursue continuing education.
• Adhere to the State Practice Act and maintain professional licensure in good standing.
• Demonstrate awareness of organizational performance improvement initiatives and follow through with action plans.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active license for the State where work will be performed.
• Previous successful assignments with Select Medical are highly desired.
