Job Description

The Outpatient Physical Therapist is responsible for evaluating assigned patients and developing, implementing, and modifying treatment plans according to patients’ needs and physician direction. This role ensures the creation of functional treatment programs that promote effective community reintegration.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans.
• Implement treatment goals focused on patient progression towards discharge.
• Document patient progress in clear, concise, and timely medical records.
• Ensure treatment is provided under signed physician orders and compliant with insurance requirements.
• Participate in student training programs as assigned, providing mentorship and supervision.
• Engage in professional development to enhance clinical skills and knowledge.
• Adhere to ethical standards and maintain licensure in good standing.
• Contribute to performance improvement initiatives to enhance quality of care.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.
• Active licensure in the state of practice.
• Previous assignment completion with the agency is highly desired.
• Experience with Select Medical is a plus.
