Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist is responsible for evaluating and developing treatment plans for assigned patients in alignment with their needs, goals, and physician direction. This role includes managing discipline-specific therapy services, ensuring effective treatment program designs that help patients return to community activities, and serving as an educator to patients, families, and staff. The position also emphasizes patient and employee safety, adherence to compliance standards, and maintaining professional conduct.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and develop appropriate treatment plans and goals.
• Document patient care clearly, accurately, and timely, ensuring compliance with relevant guidelines and policies.
• Participate in student training programs by supervising and evaluating students appropriately.
• Create and pursue an individualized professional development plan with clinical management.
• Maintain licensure and comply with state-mandated continuing education requirements.
• Engage in performance improvement initiatives and provide feedback for maximizing outcomes.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• A minimum of 1 year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure as a Physical Therapist.
• Previous experience with Select Medical is preferred.

Schedule

This position follows a 5x8.0 schedule.
