Job Description

The Outpatient Physical Therapist is responsible for evaluating assigned patients and developing, implementing, and modifying treatment plans in accordance with patient needs, goals, and physician direction. This role includes delegating and directing all discipline-specific therapy services for assigned patients while ensuring the design of functional treatment programs that support effective community reintegration.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Develop appropriate treatment plans and goals to facilitate patient progress towards discharge.
• Document medical records in a clear, concise, accurate, and timely manner.
• Provide educational support to patients, families, students, and coworkers.
• Participate in student programs as appropriate and provide guidance and evaluations.
• Maintain professional licensure and complete state-mandated continuing education requirements.
• Demonstrate compliance with organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Bachelor's or Master's degree in Physical Therapy from an accredited institution.
• A minimum of one year of outpatient clinical experience is required.
• Current CPR certification.
• Active and applicable state license for practicing as a Physical Therapist.
