Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans according to the patient’s needs, goals, and physician directions. The role requires ensuring the design of functional treatment programs aimed at facilitating effective patient return to community activities.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals.
• Document medical records in a clear, concise, accurate, and timely manner.
• Ensure care is provided under valid physician orders and within insurance approval.
• Participate in student programs, providing supervision and evaluations.
• Create individualized professional development plans with clinical management.
• Adhere to discipline-specific state practice acts and maintain active licensure.
• Engage in performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Minimum of 2 years of outpatient clinical experience.
• Current CPR certification.
• Active applicable license for the state where work will be performed.
