Job Description

The Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients’ needs and physician directions. Responsible for delegation and direction of all discipline-specific therapy services for those assigned patients, the therapist ensures the design of functional treatment programs allowing for effective return to community activities. This role also serves as an educator of patients, families, students, coworkers, and self, while demonstrating behaviors consistent with the organization's Mission Statement and Core Values.

Responsibilities

• Completes comprehensive evaluations of assigned patients and develops appropriate treatment plans and goals.
• Collaborates with the interdisciplinary team to address all barriers to discharge.
• Manages individual, concurrent, and group treatment sessions based on patient care needs.
• Ensures clear, concise, accurate, and timely medical record documentation in compliance with guidelines.
• Participates in the student program, providing supervision and development of learning objectives.
• Actively pursues professional development through continuing education and involvement in professional activities.
• Adheres to discipline-specific State Practice Act and maintains professional licensure in good standing.

Required Experience / Certifications / Licensure

• BLS Certification Required Prior to First Shift
• Must hold a valid, current State license
• Minimum of 2 years of experience
• Inpatient Rehab Experience preferred
