Case Manager

Job Description

The Case Manager is responsible for utilization reviews, resource management, discharge planning, treatment plan management, and financial management while also completing medical record documentation. This role emphasizes patient and employee safety, aiming to create a secure environment for all stakeholders.

Responsibilities

• Identify the patient's issues that affect their treatment plan and support patients and families in utilizing hospital resources.
• Assist patients and families in decision-making regarding their care plans and develop safe discharge plans.
• Collaborate with the interdisciplinary team to manage and direct clinical plans for timely discharges.
• Ensure compliance with medical necessity review criteria and timely completion of utilization reviews.
• Maintain a community resource system and facilitate access to appropriate services for patients and families.
• Document care plans and progress notes in accordance with policy and state regulations.
• Conduct patient and team family conferences following policy protocols.
• Communicate effectively with payors and address any billing or payment concerns promptly.

Required Experience / Certifications / Licensure

• Current licensure in a clinical discipline per state guidelines (RN, RT, SW preferred).
• Working knowledge of the insurance industry and government reimbursement.
• Minimum of 2 years of experience in Case Management and Discharge Planning in an Acute Care setting.
