Outpatient Physical Therapist

Job Description

This is a 13-week contract position for an outpatient physical therapist. The role involves evaluating assigned patients and developing, implementing, and modifying treatment plans according to patient needs, goals, and physician directions. Responsibilities include ensuring functional treatment programs are designed for effective community reintegration while serving as an educator for patients, families, and peers.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and utilize clinical reasoning for treatment planning.
• Complete medical record documentation accurately and timely, including goal writing and functional quality indicator assessments.
• Provide care under signed, valid physician orders and within insurance approval.
• Participate in student programs as necessary, offering a positive learning environment and thorough evaluations.
• Create a professional development plan in collaboration with clinical management, pursuing continuous education and professional activities.
• Maintain licensure in good standing and adhere to state practice acts and professional codes of conduct.
• Participate in performance improvement initiatives and offer feedback for quality enhancement.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• 6 months to 1 year of outpatient clinical experience.
• Current CPR certification.
• Active physical therapy license valid in the state of practice.
• Prior experience with Select Medical is a plus.

Schedule

This position operates on a 5x8.0 shift schedule with guaranteed hours, spanning a total of 40 hours across the assignment duration.
