Job Description

The Outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. Responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, the Physical Therapist ensures the design of functional treatment programs allowing for effective return to community activities. This role serves as an educator for patients, families, students, coworkers, and self while upholding the organization's Mission Statement and Core Values.

Responsibilities

• Completes comprehensive evaluations of assigned patients and develops appropriate treatment plans focused on progressing patients towards discharge.
• Documents medical records clearly, concisely, accurately, and in a timely manner.
• Ensures care is provided under signed, valid physician orders and within insurance approval for clinical care.
• Participates in the student program, providing supervision, support, and evaluations.
• Actively creates and pursues a professional development plan with clinical management.
• Adheres to discipline-specific State Practice Acts and maintains professional licensure and continuing education requirements.
• Demonstrates awareness of organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ years of outpatient clinical experience.
• Active CPR certification.
• Maintenance of an active and applicable state license for practice.

Schedule

This position offers a 5x8.0 schedule, with shifts that may vary.

Assignment Duration

This is a 13-week contract position.
