Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans according to the patients' needs, goals, and physician direction. This role involves the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate an effective return to community activities. The therapist serves as an educator for patients, families, students, coworkers, and themselves, performing various professional duties related to patient care and the development of discipline-specific therapy services.

Responsibilities

• Complete a comprehensive evaluation of assigned patients and develop appropriate treatment plans and goals.
• Document medical records in a clear, concise, accurate, and timely manner, ensuring compliance with state, federal, and organizational guidelines.
• Participate in student programs and provide a positive learning environment.
• Create an individualized professional development plan and pursue continuing education and professional growth.
• Adhere to discipline-specific State Practice Act and maintain professional licensure in good standing.
• Demonstrate awareness of organizational performance improvement initiatives and follow through with action plans.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an Accredited School (Required)
• 1+ year of outpatient clinical experience (Required)
• CPR Certification (Required)
• Active and applicable license for the state where work will be performed (Required)
• Previous successful assignments with our agency or Select Medical is highly desired but not mandatory.

Schedule

This position operates on a 5x8.0 shift and is a 13-week contract position.
