Job Description
This position provides expertise and leadership to ensure effective resource management for patient care delivery. The Case Manager enhances continuity of care and cost effectiveness by integrating the functions of case management, resource management, clinical documentation management, and discharge planning. Daily and concurrently involved in the management of inpatient and observation patients through comprehensive assessment, planning, interventions, and evaluations of appropriate levels of care.
Responsibilities
- Drive throughput process by ensuring safe discharge plans.
- Monitor length of stay (LOS) compared to General Medical Length of Stay (GMLOS) for reduction in opportunity days.
- Assess assigned patient population for medical necessity, level of care, and appropriateness of setting and services.
- Collaborate with Social Workers to proactively arrange discharge plans that meet facility target discharge times.
- Utilize Milliman/Interqual Care Guidelines and MIDAS systems for tracking and reporting variances and care standards.
- Demonstrate knowledge of regulatory agencies to ensure compliance with payer guidelines.
- Participate in daily interdisciplinary patient care rounds to collaboratively formulate appropriate discharge plans.
- Identify and report variances in patient care standards to support outcomes management processes.
Required Experience / Certifications / Licensure
- Graduate of an accredited school of Nursing required; BSN preferred.
- Current RN license in the State of Texas (or compact) is required.
- Minimum of 5 years in clinical practice is required.
- Experience in Case Management and Discharge Planning is preferred.
- Familiarity with MIDAS and Meditech systems is preferred.
- Bilingual (English/Spanish) is preferred.
- Certification in Case Management is preferred.
