Job Description
Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care, developing appropriate plans of care in line with regulatory and clinical practice standards. This position utilizes standardized assessments and evidence-based practices to support clinical interventions and administers various procedures as part of the rehabilitation plan.
Responsibilities
- Administers manual techniques, ambulation, therapeutic exercises, modalities, and wound care as part of the rehabilitation plan.
- Assesses patient progress and modifies treatments as needed, documenting changes in compliance with regulatory and clinical requirements.
- Evaluates treatment outcomes to ensure effectiveness and adjusts services accordingly.
- Provides consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
- Communicates effectively with supervisors and healthcare team members about patient progress, barriers, and treatment plans.
- Prepares comprehensive discharge summaries as required by regulations.
- Documents care and maintains patient records in accordance with company policy and all regulatory requirements.
- Participates in required meetings such as patient care conferences and rehabilitation meetings.
- Supervises and supports physical therapist assistants and rehabilitation aides/students as needed.
- Adheres to APTA and state-required practice acts and code of conduct during professional practice.
Required Experience / Certifications / Licensure
- 1 year of Physical Therapist experience is required; new graduates are welcome if they have completed a Level II clinical experience.
- Experience in long-term care or rehabilitation settings is required.
- Current Oklahoma State Physical Therapist License is required.
Schedule
This position involves 8-hour days with a day shift schedule.
