Description
Job Description
The Outpatient Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans according to patient needs, goals, and physician direction. The therapist will be responsible for the delegation and direction of all discipline-specific therapy services for assigned patients and ensure the design of functional treatment programs that allow for an effective return to community activities.
Responsibilities
- Perform comprehensive evaluations of assigned patients.
- Develop individualized treatment plans and goals focused on progressing patients toward discharge.
- Document all aspects of patient care in a clear, concise, accurate, and timely manner.
- Ensure care is provided under signed, valid physician orders and within insurance approval.
- Participate in student programs by providing supervision and evaluations.
- Create a professional development plan with clinical management and actively pursue continuing education.
- Adhere to regulatory guidelines and maintain professional licensure in good standing.
Required Experience / Certifications / Licensure
- Degree in Physical Therapy from an accredited school.
- 6 months to 1+ year of outpatient clinical experience.
- Current CPR certification.
- Active license for the state in which work will be performed.
- Previous assignments with the agency are highly desired.
Schedule
This is a full-time position with a 5x8.0 shift and an expected workload of 40 hours per week for a 6-month contract duration, starting as soon as possible.
