Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies their treatment plans according to individual needs, goals, and physician direction. Responsibilities include the delegation of therapy services, ensuring the design of functional treatment programs for effective community reintegration, and serving as an educator for patients, families, and team members. Adherence to professional standards and compliance with the organization's Code of Conduct is essential.

Responsibilities

• Conduct comprehensive evaluations of patients to inform treatment plans.
• Document medical records accurately and timely in compliance with regulations.
• Provide therapy services under valid physician orders, ensuring insurance compliance.
• Engage in student education and mentoring as needed.
• Maintain professional licensure and adhere to continuing education requirements.
• Participate in performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• 1 year of outpatient clinical experience preferred.
• Current CPR certification.
• Active license for practicing physical therapy in the relevant state.
