Job Description
The MDS Coordinator is responsible for completing assessments and managing care plans in a Skilled Nursing Facility. This role requires extensive knowledge of the MDS process and ongoing interaction with residents in need of short-term rehabilitation. The ideal candidate will work in a team-oriented environment, performing comprehensive evaluations and ensuring adherence to regulatory standards.
Responsibilities
- Complete MDS assessments and develop baseline care plans.
- Conduct care plan meetings and facilitate care plan development.
- Manage the submission and validation of assessments.
- Oversee certification and recertification processes.
- Educate team members on MDS changes and conduct utilization reviews.
- Review admission referrals and coordinate preauthorization for insurance.
Required Experience / Certifications / Licensure
- Bachelor of Science in Nursing (BSN) required.
- Minimum of 3 years of current MDS experience is required.
- Current NC state nursing license required.
- BLS certification required.
- Experience in Long Term Acute Care/Skilled Nursing settings preferred.
