Schedule

Day shift, 5x8-hour schedule (08:00 AM - 04:30 PM). This facility cannot accommodate 4x10s or 3x12s.

Job Description

We are seeking a dedicated Physical Therapist to join our team in a Long Term Care/SNF setting. The ideal candidate will provide skilled physical therapy services to a diverse patient population, focusing on rehabilitation and recovery. This role requires a commitment to quality patient care and collaboration with a multidisciplinary team.

Responsibilities

• Perform evaluations and develop individualized treatment plans for patients.
• Implement therapeutic interventions and monitor patient progress.
• Document all patient care activities in the electronic charting system.
• Collaborate with nursing staff and other healthcare professionals to ensure effective patient care.
• Educate patients and families on treatment plans and wellness strategies.

Required Experience / Certifications / Licensure

• Minimum of 1 year of Skilled SNF and/or LTC experience.
• Current BCLS certification required.
• Must have experience with central line blood draws, dressing changes, and IV management.
• Travel experience of at least 1 year preferred.
• No first-timers accepted; must have prior experience in a similar role.
• Must float between like units as required.
