Job Description

We are seeking a dedicated Outpatient Physical Therapist to join our team for a 6-month contract starting August 10th. This position involves evaluating assigned patients, developing and modifying treatment plans according to their needs and physician direction, and providing high-quality care to facilitate effective return to community activities.

Responsibilities

• Conduct comprehensive evaluations and create individualized treatment plans for assigned patients.
• Perform 60-minute evaluations and follow-up sessions every 30 minutes.
• Document all medical records accurately and in a timely manner, compliant with guidelines.
• Ensure all services are provided under valid physician orders and insurance approvals.
• Participate in student programs to foster learning and professional development.
• Maintain professional licensure and adhere to the State Practice Act and other regulatory standards.
• Actively engage in continuous professional development and quality improvement initiatives.

Schedule

This is a 5x8.0 shift position, emphasizing patient safety and quality of care.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• Minimum of 6 months to 1 year of outpatient clinical experience required.
• Current CPR certification.
• Active and applicable state license for Physical Therapy.
