Job Description
The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans according to each patient’s needs, goals, and physician directives. The therapist is responsible for the delegation and direction of all therapy services for those patients, ensuring the design of functional treatment programs that allow effective return to community activities. This role includes serving as an educator to patients, families, students, coworkers, and oneself. Adherence to the organization’s Mission Statement and Core Values is expected, along with compliance with safety and professionalism standards.
Responsibilities
- Complete comprehensive evaluations and develop treatment plans focused on patient discharge.
- Document medical records in a clear, concise, and timely manner, ensuring compliance with state, federal, and organizational guidelines.
- Participate in student programs, providing supervision and evaluations as appropriate.
- Create an individualized professional development plan and pursue ongoing education and involvement in professional activities.
- Adhere to state regulations and maintain active licensure in good standing.
- Understand and participate in organizational performance improvement initiatives.
Required Experience / Certifications / Licensure
- Degree in Physical Therapy from an accredited school.
- 6 months to 1 year of outpatient clinical experience is required.
- Current CPR certification is required.
- Active and applicable state licensure as a Physical Therapist is required.
- Previous assignments with our agency or Select Medical are highly desired.
