Schedule

Day shift, Monday to Friday from 08:00 AM to 04:30 PM

Job Description

As a Registered Nurse in Home Health, you will provide essential nursing care to patients primarily in York and Adams counties. Your role will involve assessment, planning, and delivering patient care in a home environment, ensuring comfort and support to patients, including those recuperating from COVID-19.

Responsibilities

Conduct patient assessments and develop care plans based on individual needs.
Administer medications and treatments in accordance with established protocols.
Perform OASIS documentation to ensure compliance and continuity of care.
Educate patients and families regarding health management and resources.
Coordinate care with interdisciplinary team members and report significant changes in patient conditions.

Required Experience / Certifications / Licensure

Minimum of 1 year of nursing experience is required.
OASIS documentation skills preferred but not mandatory.
Current RN license in Pennsylvania is required.
COVID-19 and flu vaccinations are mandatory, with room for medical or religious exemptions.
