Job Description

We are seeking a dedicated Outpatient Physical Therapist with a focus on orthopedic and geriatric patients. The successful candidate will evaluate patients, develop individualized treatment plans, and work collaboratively with healthcare teams to ensure optimal patient outcomes. You will be responsible for ensuring safe and effective delivery of therapy services while adhering to compliance standards and organizational values.

Responsibilities

• Conduct comprehensive evaluations and develop personalized treatment plans for patients.
• Implement and modify treatment programs in coordination with patient needs and physician direction.
• Provide education to patients, families, and team members.
• Document patient care clearly and accurately in compliance with all regulatory guidelines.
• Adhere to safety protocols and promote patient and employee safety.
• Participate in student programs, providing guidance and evaluations as necessary.
• Engage in ongoing professional development through continuing education and professional activities.

Required Experience / Certifications / Licensure

• Bachelor's or Master's degree in Physical Therapy from an accredited institution.
• Minimum of 1 year of outpatient clinical experience.
• CPR certification required.
• Active and valid state physical therapy license.
• Experience with Select Medical or similar agencies is a plus.

Schedule

This position follows a 5x8.0 schedule with 8-hour shifts. Ideal candidates are available to start ASAP and are open to long-term assignments, with approximately 9-10 visits per day.
