Job Description
We are seeking an experienced Registered Nurse for a home health opportunity. The ideal candidate will possess strong clinical skills and demonstrate competence in providing patient care in a home environment.
Responsibilities
- Conduct skilled home visits for patients with various diagnoses including med/surg, cardiac, orthopedic, post-surgical, and respiratory conditions.
- Perform evaluations, assessments, and create individualized care plans for patients.
- Manage medication administration and provide patient education.
- Document patient care and update health records as per agency protocols.
- Collaborate effectively with interdisciplinary teams for coordinated patient care.
Required Experience / Certifications / Licensure
- Minimum of 1 year of nursing experience required, preferably in home health.
- Current registered nurse license in Florida or compact state license.
- Basic Life Support (BLS) certification required.
- OASIS training preferred.
- Must have a valid driver's license and access to a car for home visits.
- COVID-19 vaccination or religious/medical exemption on file is mandatory.
- Flu shot required or a declination form; mask-wearing while at work is mandatory.
