Job Description
We are seeking an Acute Care Rehabilitation Therapist to join our team. The ideal candidate will have experience in various patient assessments and rehabilitation interventions within an acute care setting, focusing on optimizing patient independence and safety.
Responsibilities
- Perform functional assessments and develop individualized plans of care for rehabilitation patients.
- Implement therapeutic interventions and monitor patient progress.
- Collaborate with an interdisciplinary team, including physical therapy and social services, to ensure comprehensive patient care.
- Advocate for patient needs and facilitate effective communication among patients and families.
- Document all patient interactions and therapy outcomes using the Epic charting system.
Required Experience / Certifications / Licensure
- Minimum of 1 year of experience in an acute care or rehabilitation setting.
- Experience with functional assessments, adaptive equipment, and patient education.
- Must possess a valid California license in hand.
- Experience with Epic charting system is required.
