Job Description

The Outpatient Physical Therapist is responsible for evaluating assigned patients and developing, implementing, and modifying treatment plans according to the patients’ needs and goals, in alignment with physician direction. This position oversees and directs discipline-specific therapy services to ensure the development of effective and functional treatment programs.

Responsibilities

• Complete comprehensive evaluations of assigned patients and develop appropriate treatment plans focused on discharge within established length of stay.
• Ensure clear, concise, accurate, and timely documentation of medical records, including goal writing and functional quality assessments.
• Adhere to signed physician orders and insurance approvals when providing care.
• Participate in supervising students as assigned, creating a positive learning environment.
• Engage in professional development by pursuing continuing education and self-study.
• Maintain state licensure and compliance with regulatory agencies and professional codes of conduct.
• Contribute to performance improvement initiatives within the organization.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Active CPR certification.
• Current and valid state licensure for Physical Therapy.
