This is a 13-week contract position.

Schedule

Monday to Friday, with a guaranteed 40-hour work week.

Job Description

The Home Health Physical Therapist will provide quality care in a home environment, focusing on evaluations and therapy interventions tailored to individual patient needs. Responsiveness to service in various counties may be required, with a central focus on achieving productivity metrics.

Responsibilities

• Conduct initial assessments and evaluations of patients.
• Develop and implement personalized treatment plans.
• Document patient progress and maintain accurate records using Homecare Homebase.
• Participate in weekly case conferences to discuss patient progress.
• Provide routine visits, 30-day re-assessments, and coordinate discharge plans.

Required Experience / Certifications / Licensure

• Home Health and Oasis experience required.
• Homecare Homebase documentation experience is necessary; consideration for training can be made without this experience for the first two weeks at a reduced rate.
• Current Physical Therapist license required.
