Outpatient Physical Therapist Contract Position

Job Description

This is a 13-week contract position for an Outpatient Physical Therapist. The role involves evaluating all assigned patients, developing, implementing, and modifying treatment plans according to patient needs, goals, and physician directives. The Physical Therapist will be responsible for delegating and directing all discipline-specific therapy services for the assigned patients and ensuring the design of functional treatment programs that facilitate an effective return to community activities.

Responsibilities

• Conduct comprehensive evaluations of patients to develop appropriate treatment plans and goals, ensuring progress toward discharge within an established length of stay.
• Maintain clear, concise, and accurate medical record documentation, including goal writing and submission of necessary documentation as per state and federal guidelines.
• Ensure care is provided under signed, valid physician orders and within necessary insurance approvals.
• Participate in student programs, facilitating a positive learning environment with appropriate supervision and development of learning objectives.
• Actively pursue professional development through continuing education, self-study, and involvement in professional activities and organizations.
• Adhere to the discipline's State Practice Act and maintain professional licensure in good standing, completing all state-mandated continuing education requirements.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months to 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable license for the state where work will be performed.
• Prior experience with Select Medical is highly desired.

Schedule

This position offers a 5x8.0 schedule with evaluations scheduled for 60 minutes, followed by follow-up sessions every 30 minutes.
