Job Description
The Physical Therapist will effectively screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care. This role involves developing appropriate plans of care that align with all regulatory and clinical practice standards. Utilizing standardized assessments and evidence-based practice to support clinical interventions is key.
The therapist will administer various procedures as part of the rehabilitation plan, including but not limited to: manual techniques, ambulation, therapeutic exercises, modalities, and wound care. Regular assessment of patient progress and adjustments to treatments are required in accordance with clinical documentation and regulatory requirements. Treatment outcomes will be evaluated for effectiveness, with modifications made based on patient evaluations.
Responsibilities
- Consult and counsel patients, families, caregivers, and other service providers regarding physical disorders.
- Effectively communicate with supervisors and other health team members about patient progress, barriers, and treatment plans.
- Provide comprehensive discharge summaries of services rendered according to regulatory standards.
- Document all required clinical information in the electronic documentation system and maintain patient records according to company policy.
- Accurately record treatment minutes for services provided in compliance with all regulations.
- Participate in meetings, including patient care conferences, rehabilitation meetings, and in-service trainings.
- Supervise and support physical therapist assistants, rehabilitation aides, and students as necessary.
- Adhere to APTA and State Required practice acts and ethics during professional practice.
Required Experience / Certifications / Licensure
- 1 year of Physical Therapy experience is required; new graduates are acceptable if they have level II experience.
- Experience in Long-Term Care (LTC) or Rehabilitation is required.
- Current Physical Therapist license in Oklahoma is required.
Schedule
This is a day shift position, working 8-hour days.
