Physical Therapist - Home Health

Job Description

The Physical Therapist is responsible for providing physical therapy services to clients in a home health setting. This role includes direct patient care, collaboration with other team members, and participation in the development of comprehensive plans of care for patients.

Responsibilities

• Provide direct patient care for clients in home health settings.
• Develop and implement individualized treatment plans.
• Utilize various rehabilitation equipment effectively, including splints, orthotics, prosthetics, and ADL equipment.
• Collaborate with healthcare team members to optimize patient care.
• Document patient progress and treatment effectiveness.
• Ensure compliance with OASIS requirements.

Required Experience / Certifications / Licensure

• 2+ years of acute nursing care and home care experience required.
• Valid Driver's License required.
• MA Physical Therapist License must be included in profile.
• Active American Heart Association BLS certification required.
• Experience with OASIS documentation is non-negotiable.

Schedule

This position requires a shift from 8:00 AM to 4:30 PM, with rotating weekend and holiday coverage as needed.

Assignment Duration

This is a 13-week contract position.
