Physical Therapist

Job Description

The Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans according to patient needs and physician direction. This role involves the delegation and direction of all discipline-specific therapy services for assigned patients, with a focus on designing functional treatment programs that facilitate an effective return to community activities. The Physical Therapist also serves as an educator for patients, families, students, and coworkers, while upholding the organization’s Mission Statement and Core Values.

Responsibilities

• Completes comprehensive evaluations of assigned patients.
• Utilizes clinical reasoning to develop appropriate treatment plans and goals.
• Collaborates with interdisciplinary teams to address barriers to discharge.
• Manages individual, concurrent, and group treatment sessions based on patient care needs.
• Documents medical records accurately and timely in compliance with guidelines.
• Participates in the student program, providing supervision and evaluation.
• Actively pursues ongoing professional development through education and activities.
• Maintains professional licensure and adheres to ethical codes of conduct.
• Implements organizational performance improvement methodologies.

Required Experience / Certifications / Licensure

• BLS Certification required prior to first shift.
• Valid and current state license as a Physical Therapist.
• Minimum of 2 years of experience in a therapy setting.
• Previous LTACH experience is preferred.

Schedule

This position operates on a Day 5x8.0 schedule.
