Schedule

AM shift, Monday to Friday, 5 × 8-hour shifts, with guaranteed 40 hours per week.

Job Description

We are seeking a dedicated Home Health Physical Therapist (PT) to join our team. The ideal candidate will provide high-quality rehabilitation services in the home setting and demonstrate a commitment to patient care. Responsibilities include conducting evaluations, establishing treatment plans, and facilitating rehabilitation programs in accordance with clinical guidelines.

Responsibilities

• Conduct patient evaluations and assessments to determine therapy needs.
• Develop and implement individualized treatment plans based on patient goals and clinical standards.
• Document patient progress and modify treatment plans as necessary.
• Collaborate with other healthcare professionals to optimize patient care.
• Provide patient and caregiver education regarding treatment plans and exercises.
• Complete required documentation in Homecare Homebase.
• Achieve productivity standards of 28-32 points per week.

Required Experience / Certifications / Licensure

• Current state licensure as a Physical Therapist (PT).
• Home Health and OASIS experience is required.
• Experience with Homecare Homebase documentation is preferred.
• Ability to travel within a 60-mile radius from the facility address.
