Rehabilitation Therapist - Skilled Nursing Facility

Job Description

We are seeking a dedicated Rehabilitation Therapist to provide comprehensive rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF). The ideal candidate will possess a current State of Hawaii license in good standing appropriate to the discipline.

Responsibilities

  • Conduct evaluations, reassessments, treatments, and clinical services as per licensure, discipline, and scope of practice.
  • Develop, implement, and manage individualized treatment plans in collaboration with the supervising therapist, physician, and interdisciplinary team.
  • Implement evidence-based therapeutic interventions to enhance function, independence, communication, mobility, swallowing, and quality of life.
  • Educate residents, families, and caregivers about treatment plans and patient progress.
  • Complete accurate and timely documentation in compliance with facility, state, federal, and payer requirements.
  • Supervise therapy assistants when applicable to the role.
  • Remain available to work during Hawaii State and Federal Holidays as required by facility operations.

Required Experience / Certifications / Licensure

  • Minimum of two (2) years of experience in the SNF/ICF setting preferred.
  • Current Basic Life Support (BLS) certification.
  • Current State of Hawaii license in good standing appropriate to the discipline.

Schedule

This position typically involves a 5-day work week with variable schedules, including one weekend day and the possibility of floating to other units or facilities based on staffing needs.

Assignment Duration

This is a 13-week contract position.
