Job Description
We are seeking a dedicated Licensed Practical Nurse (LPN) for a Long-Term Care facility with an average daily census of 21 patients. You will work in a setting accommodating 23 beds, focusing on providing exceptional patient care in collaboration with an interdisciplinary team.
Responsibilities
- Manage patient care, including medication administration and treatment compliance.
- Conduct patient assessments and monitor vital signs.
- Assist with daily living activities and provide emotional support to patients and their families.
- Collaborate with a multi-disciplinary team to develop and implement care plans.
- Ensure adherence to safety and regulatory standards.
Required Experience / Certifications / Licensure
- A minimum of 1 year of recent LPN experience is required.
- Previous charge experience is required.
- BLS certification is required.
- Must possess a valid NY state license to practice as an LPN.
- Experience with the EPIC charting system is a hard requirement.
