Physical Therapist / Occupational Therapist / Speech-Language Pathologist

Monday - Friday, 5 × 8 hour shifts

Job Description

We are seeking dedicated Physical Therapists, Occupational Therapists, or Speech-Language Pathologists to provide high-quality care in our home health setting. The role involves visiting patients throughout Burnet, Bertram, and Lampasas, and may require coverage in other territories within a 60-mile radius as needed.

Responsibilities

• Conduct 4-6 patient visits per day based on the type of therapy required.
• Achieve a productivity benchmark of 28-32 points per week for therapy disciplines.
• Document services using Homecare Homebase system.
• Perform evaluations, treatment plans, and follow-up assessments as necessary.
• Participate in weekly case conferences and team meetings.

Required Experience / Certifications / Licensure

• Home Health and OASIS experience required.
• Experience with Homecare Homebase documentation preferred.
• Licensure in the relevant field is mandatory.
