Occupational Therapist - Home Care - Full Time, Days

Job Description

As an Occupational Therapist at our organization, you will have the unique opportunity to forge meaningful connections with your patients and become a vital part of their care team. With an average visit time of over 60 minutes, you will be able to provide comprehensive therapy that truly impacts their lives. You will enjoy a balance of autonomy and teamwork, working alongside a dedicated health care team that cares about delivering quality services.

Your role will include assessing, evaluating, re-evaluating, and treating patients, as well as coordinating and communicating treatment activities with other team members and the patients themselves.

Responsibilities

• Assess the patient's functional ability.
• Provide direct treatments.
• Select appropriate therapeutic modalities.
• Communicate with all members of the health care team.
• Document evaluations, treatment goals, and objectives.
• Educate patients and family members on treatment rationales and procedures.

Required Experience / Certifications / Licensure

• Must be a licensed Occupational Therapist in Vermont and New Hampshire or hold an unencumbered compact license.
• BS in Occupational Therapy or completion of a postgraduate Occupational Therapy Certification program.
• Must have passed the National Registration Exam.
• Experience in a home health setting is preferred.

Schedule

This position requires a full-time commitment, with days as the primary shift.
