Physical Therapist - Day Shift

Job Description

The Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient's needs and physician direction. This role is responsible for delegating and directing all discipline-specific therapy services for assigned patients and ensuring the design of functional treatment programs for effective community reintegration. As an educator, the Physical Therapist provides support to patients, families, students, and coworkers while demonstrating behaviors consistent with the organization's mission and core values, and maintaining compliance with the organizational code of conduct.

Responsibilities

• Complete comprehensive evaluations of assigned patients and develop appropriate treatment plans and goals.
• Collaborate with the interdisciplinary team to address all barriers to discharge, including timely equipment trials and family training.
• Manage individual, concurrent, and group treatment sessions based on patient care needs.
• Document medical records accurately and in a timely manner, adhering to state and federal regulations.
• Participate in clinical supervision and mentoring of students when assigned.
• Create an individualized professional development plan and pursue ongoing professional growth opportunities.
• Adhere to state practice acts and maintain professional licensure in good standing.
• Understand and follow the organization’s performance improvement methodology.

Required Experience / Certifications / Licensure

• Valid and current state license as a Physical Therapist.
• Minimum of 2 years of experience in a relevant setting, such as inpatient rehabilitation or neuro experience is highly desired.
• BLS certification required prior to first shift.

Schedule

This is a full-time day shift position, with a schedule of five 8-hour shifts.
