Job Description
CVHCare is seeking a qualified Physical Therapist to deliver exceptional patient care in designated territories. The position requires effective communication and collaboration with patients and healthcare providers.
Responsibilities
- Conduct evaluations and create treatment plans for patients.
- Provide routine therapy sessions and assessments.
- Document patient progress and adjust treatment plans as necessary.
- Utilize OASIS assessments where applicable.
- Maintain productivity standards and caseload expectations.
Required Experience / Certifications / Licensure
- Current California Physical Therapist License
- BLS certification
- Valid Driver's License and auto insurance
- TB screening (annual), MMR, Varicella, HepB, TDap (10 years), Flu (annual)
- Drug screen and physical examination
