Days: 8am-5:30pm Monday - Thursday, 8am-12pm Friday

Job Description

This position focuses on patient care and utilizes assessment, planning, intervention, and evaluation in accordance with physical therapy standards, physician orders, and governing policies and procedures. The Physical Therapist will delegate responsibilities to the Physical Therapist Assistant and Aides.

Responsibilities

• Evaluates the patient’s function through formal/informal testing measures to determine skeletal, neurological, respiratory, and cardiovascular status.
• Develops a goal-specific treatment regime based on assessment data and the patient’s tolerance to carry out the treatment plan as prescribed.
• Submits a written treatment regime to be included as part of the plan of treatment authorized by and coordinated with the physician. Updates plan of care as needed to reflect changes in patient status.
• Devises a schedule of treatment that is frequency and duration specific, discussing it with the patient/family/caregiver and coordinating with clinical support staff.
• Educates the patient, family, and/or caregiver regarding pathology or condition, treatment alternatives, prognosis, and rationale for treatment through various interventions.

Required Experience / Certifications / Licensure

• PT Experience: Required
• Outpatient Experience: Required
• Active American Heart Association BLS Certification: Required
• Active Michigan Physical Therapist License: Required
• Two Supervisory Reference Forms completed within the last two years: Required
• Skills Checklist, current to 1 year, pertinent to the specialty (Physical Therapy): Required
